Key Responsibilities:
- Develop and oversee individualized social service care plans, supporting residents' emotional and social well-being
- Act as an advocate for residents, ensuring their needs and concerns are addressed and respected
- Coordinate with healthcare providers, family members, and community resources to support comprehensive care
- Ensure compliance with state and federal regulations and maintain accurate documentation
- Lead and mentor the social services team, promoting a collaborative and compassionate care environment
Qualifications:
- Education: Bachelor's degree in Social Work or a related field
- Experience: Prior experience as a Social Services Director or similar role in a healthcare or post-acute setting
- Licensure: Licensed Social Worker in Alaska (or eligibility to obtain licensure)
- Compassionate Leader: Demonstrated commitment to resident-centered care and strong interpersonal skills
- Organized and Detail-Oriented: Excellent organizational skills and the ability to manage documentation and compliance requirements effectively
