Community Health Worker (Healthcare)



Job Summary: The Community Health Worker Apprentice assists individuals and communities to adopt healthy behaviors. This position conducts outreach for the Community Health Hub to implement programs that promote, maintain, and improve individual and community health. The individual will provide information on available resources, provide social support and informal counseling, advocate for individuals and community health needs as well as assist in data collection to help identify community health needs.

Essential Functions:

  • Identify the health care needs of residents in the City of Hagerstown.
  • Maintain updated program participant records with plans, notes, appropriate forms, or related information.
  • Advise program participants or community groups on issues related to sanitation or hygiene, such as flossing or hand washing.
  • Advise program participants or community groups on social or intellectual development issues, such as education, childcare, or problem-solving.
  • Advise program participants or community groups to ensure parental understanding of the importance of childhood immunizations and access to immunization services.
  • Attend community meetings or health fairs to understand community issues or build relationships with community members.
  • Contact program participants in person, by phone, or in writing to ensure they have completed required or recommended actions.
  • Distribute flyers, brochures, or other informational or educational documents to inform members of a targeted community.
  • Provide feedback to health service providers regarding improving service accessibility or acceptability.
  • Refer community members to needed health services.
  • Assist families in applying for social services, including Medicaid or Women, Infants, and Children (WIC).
  • Collect information from individuals to compile vital statistics about the general health of community members.

Minimum Requirements:

  • Community Health Worker Apprentice must be enrolled in Community Health Worker Certification Program offered by AHEC West.
  • Lived experience as a recipient of social services
