Provide services to youth on Probation as needed, which may include conducting rehabilitative services including individual or group services to consumers. Conduct or be present for CFTs as needed for assigned cases. Meet with and provide case management services to the families of probation involved youth. Provide linkage, resources and education as needed to the consumer and support systems. May need to provide co-occurring rehabilitative services individually or in group for Probation consumers or other rehabilitative services or resources that could be helpful for the consumer, and/or family. Assist with coordination or pertinent treatment information and communication between treatment team in various meetings in the clinic, at schools, probation or other agencies involved in consumers treatment, etc. At times, may need to provide services at satellite clinics, schools, or homes when needed such as Avenal, Corcoran, Stratford, Kettleman, etc. There may include some evening hours based on program need.
How will I contribute
Advocate for and assist probation involved youth and adolescents and their families in accessing and receiving mental health service, medical care, housing, educational services, food, and clothing as indicated. Work closely with Mental Health Clinician to implement individual care plans for consumers. Provide guidance and serve as a role model to consumers in reaching their goals and assist in monitoring progress toward achieving those goals. Develop and provide rehabilitative skills groups by developing cohesive group curriculum and facilitating or co-facilitating skills groups. Communicate with and translate for Spanish speaking consumers, if applicable. Consult with and meet regularly with consumers family or primary care providers, to review consumers progress, obstacles and resources needed by consumers. Assist in establishing consumers eligibility for Medi-Cal or other benefits, and advocate for the continuation of such benefits when appropriate. Actively involve parents/families/care providers in planning and treatment decisions, for their children. Meet with primary clinician as assigned on a weekly basis or at least bi-weekly basis to discuss new referrals and progress, on assigned individual and group referred cases. Conduct home and school observations when needed in order to collect all necessary child and family history support of medical necessity, as to develop appropriate behavioral intervention. Participate in weekly supervision with immediate supervisor and attend all scheduled department meetings. Maintain positive working relations with community agencies. Participate in CFTs and collaborative meetings when needed. Achieve basic competence in using our electronic record keeping system. Complete documentation with 3 business days of service. Achieve basic competence in using One Point, or electronic time card system. Maintain a yearly productivity standard. In the course of performing job duties, follows all Kings View policies and procedures, administrative directives, all relevant county, state and federal regulations, and any performance obligations contained with Kings View program contracts with other entities. Achieve all performance standards. Maintain familiarity with and adherence to compliance with the code of conduct and the requirements of the compliance program that are elements of performance evaluations and conditions of employment. Perform other duties as assigned.
What I bring
A. degree plus 4 years experience, or LPT/LVN/RN plus 4 years experience, or B.A. degree plus 2 years experience, or 4-year degree in activity therapy or related field from a college or university plus registration or certification with the respective professional organization and 2 years experience Class C Driving License Expertise in program services delivery practical and theoretical. Data entry into various electronic systems while maintaining the integrity and accuracy of the data. Relate to others comfortably and able to establish and maintain effective relationships. Strong verbal, written, and interpersonal skills -Integrity to handle sensitive information in confidential manner Action oriented. Strong problem-solving skills. Excellent organization skills and ability to multitask and juggle multiple priorities. Outstanding ability to follow-through with tasks. Flexible and reliable. Bilingual preferred
