LVN (Healthcare)
Patient Care:
Administer medications and treatments as prescribed by the healthcare provider.
Monitor patients' vital signs and report any changes in condition to the RN or physician.
Assist in developing and implementing individualized care plans for residents.
Provide basic nursing care, including wound care, catheter care, and assisting with daily living activities.
Ensure patient comfort and safety by responding promptly to call lights and addressing patient needs.
Documentation and Communication:
Accurately document patient care, observations, and interventions in the medical record.
Maintain open and effective communication with patients, families, and the healthcare team.
Participate in shift handoffs to ensure continuity of care.
Team Collaboration:
Work collaboratively with RNs, CNAs, and other healthcare professionals to deliver comprehensive care.
Assist with the training and orientation of new staff members as needed.
Participate in care conferences and contribute to the interdisciplinary team's decision-making process.
Compliance and Safety:
Adhere to all facility policies, procedures, and regulations, including those related to infection control and resident rights.
Ensure proper use of medical equipment and report any malfunctions to appropriate personnel.
Maintain a clean, safe, and organized work environment.
Qualifications:
Education:
Graduate of an accredited LVN program.
Licensure:
Current and valid LVN license in the state of California.
Experience:
Prior experience in a skilled nursing or post-acute care facility is preferred.
Certifications:
BLS (Basic Life Support) certification required.
Skills:
Strong clinical and critical thinking skills.
Excellent communication and interpersonal skills.
Ability to work effectively in a fast-paced environment.
Proficiency in electronic medical records (EMR) systems is a plus.
Physical Requirements:
Ability to lift and move patients as required.
Prolonged periods of standing and walking.
Ability to perform physically demanding tasks, such as repositioning patients.
